Medical questions translated to layman terms for patients sets the tone and pace to ensure patients complete their questionnaire i.e. intake form with ease.

After watching numerous patients go through the questionnaire, we noticed patients did not know there were more questions following after the previous one nor did they realize to scroll down or where to tap. Animated arrows, helper text such as “Scroll down or Tap this” gave them the direction they needed to complete the questionnaire.

The first initial design was to have the patients free text their medication and allergies. That failed as patients did not want to type all 10+ medications nor did they remember the spelling. Free text turned out to be “Just ask my doctor.”

We created a type ahead medication list so patients only needed to type in the first letter or more and to select their medication or allergies from the list.

Iconography

The initial design for the review of systems (ROS) did not include any iconography. We initially thought patients would be able to go through each of these with ease but what we observed were patients had to think about which of their body parts had issues. It was not clear even with words like “Urination” or “Nervous System”.

Once we added some visual treatment, patients started to understand which question was for which body part and even placed their hands on their eyes or chest to indicate what to select.

EMR Integration

EMR Integration

Once the questionnaire is completed, it automatically syncs to an EMR i.e Athena Health. Doctors are able to scan through the notes prior to the patient’s visit which saves time and allows more face time with the patient.

OUTCOME: Completion rates went up from 81.2% to 92%.

LEARNINGS: We spent a lot of time with doctors getting Athena Health to look the way they wanted. Less customization up front would have made us get to market faster.

GROWTH: Provide an off-the-shelf dashboard along with a foundation of questions for each practice. Iterate on data collected and plan for customization.
